2- DOWNLOAD REGISTRATION FORM BELOW AS PDF FORMAT or WORD DOC.
SEND COMPLETED REGISTRATION FORMS VIA EMAIL TO: email@example.com
When you email your form include your NAME AND PHONE NUMBER
in the body of the email in case we have difficulty reading the form.
FCPS and WPS students sending the form with an email ending in fcpsk12.net or the wps.k12.va.us (student) are not able to send attachments.IF you are using a fcpsk12.net or a wps...student email choose one of the other options.
3- COMPLETE THE FORM ON THE BOTTOM OF THIS PAGE AND SUBMIT.
If you DO NOT receive a reply to your email with the registration form within 24 hours
call 540 398-6059 because we did not receive your registration if you do not hear from us.
